I'm sure it has happened before but it has to be fairly unique. I know that it happened at least once in MLB since 2016. However, I believe that it is the first walk-off hit by pitch for the Mets since 2014.

Conforto, you’re better than that. I blame the ump for calling it. Catchers frame balls that get called strikes all the time. For decades, you only had to sorta be near second base when turning a double play. Conforto tried to get away with something for sure, but the ump shouldn't have awarded him first base.
